Yes, in order to verify your identity and the state in which you reside, we will need you to fax your ID or driver' license to our secure EHR via fax at 1-866-531-4061.
If you will be using your insurance card to pay for labs, then we will need a copy of your insurance card sent to our secure EHR via fax at 1-866-531-4061. Insurance information is needed to place an order with labcorp and quest diagnostics. However, if you are paying cash for your labs then you will not need your insurance card.
Please call us at 302-412-5099 and let us know that you would like to become a client. If you receive a voicemail leave us a detailed message and inform us of the best times to reach you. We will call you back to briefly discuss your goals. Before becoming a client we will need you to fax over any necessary records to our secure EHR at 1-866-531-4066; once we receive your records we will inform you and will contact you to schedule an intial consultation appointment. After your first visit, you will receive a Welcome Letter with next steps and a login to our EHR (Electronic Health Record), which you can access on the menu at the top of this page.

We do not take currently accept insurance but dont let that stop you. We have other options for payment plans please give us a call.
